Visit any NHIF office or Huduma Center near you with a copy of your
ID, your spouse's ID copy, marriage certificate copy and in case you
do not have one an affidavit from the Magistrates' court and NOT FROM
AN ADVOCATE or A LAWYER. Last but not least, for the children copy of
their birth certificates or birth notification if the child is between
0-6 months and passport size photos for all dependants.
